Celiac disease also known as sprue, nontropical sprue, and gluten-sensitive enteropathy is a digestive disorder caused by an abnormal immune reaction to gluten. The results of the meta-analysis found the current worldwide prevalence of celiac disease to be 1.4% based on blood tests and 0.7% based on biopsy results. The prevalence of celiac disease was 0.4% in South America, 0.5% in Africa and North America, 0.6% in Asia, and 0.8% in Europe and Oceania.

Gluten is a protein found in foods made with wheat, barley, rye, and triticale. It is also found in oats that have been made in processing plants that handle other grains. Gluten can even be found in some medicines, vitamins, and lipsticks. Gluten intolerance, also known as gluten-related disorders, is characterized by symptoms after eating gluten-containing foods. People with non-celiac gluten sensitivity may have a mild sensitivity to gluten, while others have celiac disease which is an autoimmune disorder.

In celiac disease, the immune response to gluten creates toxins that destroy the villi. Villi are tiny finger-like protrusions inside the small intestines. When the villi become damaged, the body is unable to absorb nutrients from food. This can lead to malnutrition and other serious health complications, including permanent intestinal damage.

According to the National Institute of Diabetes and Digestive and Kidney Diseases, about 1 in 141 Americans has celiac disease. People with celiac disease need to eliminate all forms of gluten from their diet. This includes most bread products, baked goods, beer, and foods where gluten may be used as a stabilizing ingredient.

What does gluten-free mean?

Gluten-free claims on packaged foods and supplements are regulated by the US Food and Drug Administration (FDA) and standards are in place requiring manufacturers to call out certain common allergens if they are included in a packaged food. Unfortunately, similar laws are not in place for labeling gluten in medications, although a proposed bill was reintroduced to Congress in September 2015.

For a food to be labeled as gluten-free, the FDA states that it must contain no more than 20 parts per million (ppm) of gluten.

The choice of 20 ppm rather than zero ppm is because current technology cannot reliably measure gluten presence below 20ppm. Evidence suggests that trace amounts of gluten, defined as up to 20 ppm, do not have adverse health effects on people with celiac disease. The use of the label is voluntary for manufacturers, but if they use it, consumers will know that their products really are gluten-free.

Avoiding gluten is important for people with celiac disease, as they can experience serious adverse effects if they consume it even in drugs and supplements. According to celiac-disease.com, the following drugs and medications ingredients are gluten-free:
